STATE OF ILLINOIS HEALTH AND MEDICAL REQUIREMENTS
Students may be excluded from school without the following requirements by the first day of school in August. All medical forms should be uploaded to FACTS by August 10; families who need assistance uploading their forms can receive help during Enrollment Support Day on August 12. Full medical physical exams must be filled out on the State of Illinois Certificate of Child Health form and signed by a physician. (*See bottom of page for sports physical requirements.)
Preschool 3 or 4 (whichever is the first year of school at QAS):
● Complete current (within the last year from first day of school) medical physical including diabetes screen
● Lead blood test – all students residing in Chicago
● All required vaccinations including:
o DTaP
o Polio (IPV)
o Hib
o MMR
o Varicella
o Hepatitis B
o Pneumococcal
Kindergarten:
● Complete current (within the last year from first day of school) medical physical including diabetes screening
● All required vaccinations up-to-date (see above)
● Lead blood test – all students residing in Chicago (unless completed in preschool at QAS)
● Eye exam
● Dental exam
2nd grade: NEW dental exam
6th grade:
● NEW dental exam
● Complete NEW medical physical including diabetes screening and all up-to-date vaccinations, including but not limited to:
o Tdap
o Meningococcal (one dose on or after 11th birthday)
Any new QAS student who has already attended an Illinois school:
Complete current medical records and/or exams corresponding to grade-level requirements (see above)
SPORTS:
*All students who plan to play sports during the school year, including intramurals, must have a sports physical (IESA/IHSA form) OR complete physical exam (State of Illinois Certificate of Child Health form) completed within the last year from the first day of school.
Additionally, parents must sign the CONCUSSION form distributed by the Athletics Department.
Sports physicals MAY NOT be used in place of any required medical exam as outlined above. However, a complete physical exam may be used in place of a sports physical.
